Earlier we have seen Virat and Pakhi face a tough time when Vinayak refuses to take medical treatment from any other doctor except Sai.
Pakhi can't see her son in pain and thus she meets Sai and requests her to treat Vinayak. Poor Sai falls into a dilemma because she doesn't want to return to Chavan's life. Thus, Pakhi reminds Sai of her promise that she made to Vinayak. And this badly impacts Sai as she feels guilty for not fulfilling her promise made to Vinayak.
